You have to call your service provider. They are the ONLY people that can give you the PUK code. Do NOT try to guess the code! If you do, you will completely block your sim card and have to buy a new sim card. They will verify that you are the account holder before giving you the PUK code. Stay on the line with them until you have successfully entered the PUK code. You can get the unlock code for free from the cell company.
A law was passed and enacated in 2006 that requires cell companies
provide you with the unlock code for free, on request. Check
out www.copyright.gov for more details on this.
They will ask you to provide them with the IMEI number of the phone
(essentially, the electronic serial number). They will email the
unlock code and instructions to you in a few days.
